For several years, I have sent a paper feedback questionnaire with every referral I make. Although only a small number are returned each year, I like to think it indicates that I am open to feedback if need be.
I am now making this available as an online form to make it even easier for referees.
(My Google account seems to prevent me from submitting this as a template. If anyone is able to do this for me - with my CC-BY attribution - I would be grateful!)
